(b) The Mayor shall establish requirements for periodic testing for lead poisoning and dental examinations.
Oral Health Assessment Form
The Oral Health Assessment Form is used to implement the law mandating periodic dental examinations for students in grades pre-K through 12. The student's dental provider should complete the form during the examination to be returned to the student's school.
